Call 224-213-0967 to book an appointment for a drop off date. Upon dropping your dog off, paperwork must be completed. Before you come in, a copy of the following veterinary records is required: 

  • Proof of Rabies Vaccine or Exemption
  • Proof of Distemper Vaccine or Exemption
  • Negative Fecal Test 
  • Any Current Medication
